Our highly-trained Care Access Pros (CAPS) can go anywhere in the United States to provide care and build relationships with members in their homes. CAPs know how to look for and document member issues beyond the screening(s) they perform - like Fall Risk, Social Determinants of Health and Health Equity concerns.
Information and results from in-home appointments are sent to our nationwide network of clinical providers, members, PCPs and our clients.

Engagement Specialists are located in the United States across all time zones, and our team is built to scale to your needs. We make roughly 3 million calls each year to members.

We send our teams into local communities to perform important screenings, provide education and build relationships. We also offer “RSVP” services to help build awareness and invite members to important health events.
